1. ADVISOR PAID PLAN (aka ANNUAL MAINTENANCE PROGRAM PLAN):*

Price:
$20.00 per Member per year (first 50 Members)
$15.00 per Member per year (next 50 Members)
$10.00 per Member per year (balance of Members)
*If an Advisor signs up at least 100 clients initially, then the price automatically drops to $10.00 per person.

Benefits: One year Membership in MPW and Permanent Plastic Member Card

Conditions: Advisor must register each Member; and Advisor (or Member) responsible for uploading documents.

Renewal: Renewal must be paid by Advisor upon expiration of each year's membership term.

Optional Add-ons: Scanning Fee $10.00 per Member

2. UNLIMITED NEW MEMBERS PLAN:

Price: $200.00 per year

Benefits: One year initial Membership to an unlimited number of New Members during the year

Conditions: Advisor must register each New Member; Member Card must be printed from our website; and Advisor (or New Member) responsible for uploading documents.

Renewal: Upon expiration of the one year initial Membership, New Member can elect to continue membership directly through MPW. Advisor cannot renew or re-add Member.

Optional Add-ons: Permanent Member Card $10.00 per card
Scanning Fee $10.00 per Member

3. PAY-AS-YOU-GO PLAN:

Price: $20.00 per New Member

Benefits: Each New Member receives permanent plastic Member Card; and
Advisor can forward documents to MPW for free scanning and uploading.

Conditions: Advisor must register each New Member

Renewal: Cannot be renewed by Advisor. Upon expiration of the one year initial Membership, New Member can elect to continue membership directly through MPW.

An explanation of our fees is provided below. In order to obtain the professional advisor pricing you agree to pay for our services on behalf of your clients. Therefore, we will bill you directly and we have no obligation to bill your clients. At least thirty days prior to the yearly renewal date (for the Advisor Paid Plan only), we will send you an invoice for your currently enrolled client for the forthcoming year. If you do not pay the invoice prior to the renewal date, we reserve the right to contact your client directly and if he or she so chooses, individually renew him or her.

1. Initial Enrollment Fee.

Your client’s initial subscription to MPW is for a term beginning on the date of his or her enrollment and ending on the last day of the twelfth month following. The following examples illustrate your client’s initial enrollment term.

The initial enrollment fee includes the creation of, and unlimited access to, your client’s Personal File.

2. Annual Renewal Fee.

Upon the expiration of your client’s initial one-year term, you may renew your client’s subscription for additional one-year terms. (NOTE: Applies to Advisor Paid Plan only)

3. Optional Scanning and Uploading Fee.

Your client’s Personal File allows him or her to store copies of their signed Health Care Documents. Once stored in your client’s Personal File, these documents can then be downloaded and printed by doctors, hospitals, and loved ones at any time.

The client stores these documents in his or her Personal File by uploading them from a computer. You can do this for your client or you can have your client do it. In order to upload these documents, they must be in one or more .pdf files. In order to create a .pdf file, you (or your client, as the case may be) will need a scanner to scan the documents into a computer and software capable of storing the documents in .pdf format.

If neither you or your client have the ability to scan and create .pdf files, then you can mail signed copies of your health care documents to us and we will create the appropriate .pdf files and upload them into your client’s Personal File. Our Optional Scanning and Uploading Fee apply each time you or your client mail documents to us.

4. Optional Fee for Additional Permanent Member Cards

For certain plans, the initial enrollment fee entitles your client to one permanent Member Card. You or your client may order additional permanent Member Cards at the time of initial enrollment, or at any time in the future.

Once paid, the initial enrollment fee is guaranteed for its one-year term. We reserve the right to change our annual renewal fees and optional fees at any time.
