MY PERSONAL WISHES HEALTH CARE PLANNING PROCESS™
Proper planning for your health care involves a four-step process:
1) Education.
The first step is educating yourself about your legal rights regarding your health care.
The United States Constitution guarantees your right to make decisions regarding your health care, including the right to refuse medical treatment. You also have the right to state in advance your desires regarding medical treatment if you are unable to consent at that time. (Click here for more information on the Education Process).
2) Documentation.
The second step is documenting your desires regarding your health care. In particular, you should consider signing the following health care documents (also known as Advance Directives):
- Living Will Declaration
- Health Care Power of Attorney
- HIPAA Authorization
- Organ Donation Authorization.
(Click here for more information on the Documentation Process).
3) Communication.
The third step is communicating your desires with your doctors and loved ones.
Studies have shown that most individuals do not talk to their doctors or family about their desires or inform them of the existence of their health care documents. This can lead to family confusion or conflict. (Click here for more information on the Communication Process.)
4) Location.
The final step is locating your health care documents and other vital medical and personal information in a place that is readily accessible in the event of your medical emergency.
That is our job! My Personal Wishes is the most comprehensive storage and retrieval service available. By having both your documents and other information available in one location, you can have the peace of mind that your doctors and loved ones will have the information necessary to ensure that your personal wishes will be carried out.
