Q: How do I become a member?
A: You may apply online to become a member. It only takes a few moments. Once your application is complete you will have access to your Personal File.
Q: How do I access my Personal File?
A: After you become a member you are assigned two numbers:
- Member Number
- Personal Password.
You can access your Personal File by typing in your Member Number and Personal Password. Your Personal Password allows you to complete and edit information stored on your Personal File at any time, day or night.
Q: What information can I store on my Personal File?
A: Your Personal File is divided into the following Sections:
- Personal Information
- Medical Information
- Family Information
- Health Care Documents
- Emergency Contacts
- Minor Children
- Adult Dependents
- Pets
At the beginning of each Section is a description of the information that you should provide. You may complete as many of these Sections as you desire. We would strongly recommend that you complete all Sections that apply to you.
Q: Is my Personal File secure?
A: Yes. The ability to complete or edit the information stored on your Personal File requires both your Member Number and Personal Password. Therefore, you should make sure that your Personal Password is stored in a secure place.
Q: Do my doctors, hospitals and other health care providers need access to my Personal File?
A: Yes. In the event of your medical emergency, these medical providers need the information stored in your Personal File.
Q: How to the medical providers access my Personal File?
A: In order to access your Personal File, your medical providers must have your:
- Member Number
- Public Password
Q: What is my Public Password?
A: Once you become a member, you are also assigned a Public Password. This password permits the viewing and printing of the information contained in your Personal File. It does not allow that information to be changed in any way. Only your Private Password permits changes.
Q: How will my medical providers know my Member Number and Public Password?
A: Once you become a member, you will receive in the mail a Member Card. Your Member Number and Public Password are on the front of your Member Card. Your Member Card also alerts the reader about the importance of your Member Card and provides our website address and phone number.
Q: Where should I keep my Member Card?
A: You should keep your Member Card with you at all times, preferably in your wallet or purse. By doing this, your medical providers will have access to your Member Card in the event of your medical emergency.
Q: How long before I receive my Member Card?
A: You should receive your Member Card in the mail in about two to four weeks.
Q: What if I am in an accident before I get my Member Card?
A: You may print out a temporary Member Card by following the directions on our website. You can place this temporary Member Card in your wallet or purse until you receive your permanent Member Card.
Q: Should I have more than one Member Card?
A: We recommend that you give an additional Member Card to at least one family member or loved one. That way, they also have immediate access to your Personal File in the event of your medical emergency.
Q: How do I get more than one Member Card?
A: You may order additional Member Cards at any time. In the alternative, you may print out temporary Member Cards and give them to your family or loved ones.
